HSA Alumni feedback survey


We encourage all Health Science graduates to inform us how to improve the service of our alumni, as this feedback will help us to plan future events and activities.

UWA's Health Science Alumni (HSA) was formed in April 2008 to offer great opportunities for Health Science graduates to network and keep in touch with previous classmates. The HSA now boasts over 100 members and continues to grow each year.

To ensure that the alumni remains relevant and interesting to you, the committee would like your assistance by completing a short survey. The survey is anonymous and you do not have to be a financial member to participate.

It will take only 10 minutes of your time (perfect for a Friday afternoon at work) and all completed entries will go into a draw to win a $50 JB HiFi voucher. To be in the running for the prize, please enter your full name and email address in the forms below:

By completing and submitting your survey responses, you are agreeing to this information being used for the sole purpose of developing the HSA. The HSA will not release any of your responses or contact details to a third party.

Please complete all questions and remember the more detailed your responses, the better your alumni will be.

All identifying entries will be submitted to an independent party for drawing of the prize. The HSA will only receive anonymous survey responses.
